All shortlisted candidates are to report for screening with the following documents (both original and photocopies):
- National Identity Card (NIN)
- Academic qualifications (HND or ND)
- O’level (WAEC/GCE/NECO or NABTEB)
- Two (2) Recent Passport Photograph
- Your completed Guarantor’s Form
- Printout of application acknowledgement
- Writing material etc.
Eligibility Criteria for LSHSC Shortlist
Here are the key criteria used by the Lagos State Health Service Commission to shortlist applicants.
- First, applicant must be a Nigerian and possess a valid National Identity Number (NIN).
- Must have submitted application before LSHSC set deadline.
- He or she must have O’level with five credit passes.
- Should hold a degree or equivalent qualification from an accredited institution.
- Must possess valid practice license (if applicable)
- Should hold NYSC discharge or exemption certificate
